AMPS Hearing Request

Request a Hearing

If you have received a Screening Decision, you may request a review of the Screening Decision by a Hearing Officer within 15 days of the Screening Decision being delivered.

If your Screening Decision was delivered more than 15 days ago, you must request a Hearing Extension.

If your Screening Decision was delivered more than 30 days ago, you are deemed to have waived the right to request a hearing. The Screening Decision, Administrative Penalty, and any Administrative Fees, if applicable as modified in the Screening Decision, shall be deemed to be affirmed and shall not be subject to any further review, including review by any Court.

Please note you are not eligible to apply for a Hearing Review if any of the following apply to you:

Hearing Extension Request Form

Guidelines

Because your Screening Decision was delivered more than 15 days ago, you must request a Hearing Extension.

You will need to explain the reason for not submitting a Hearing Request before the deadline.

The Hearing Officer may only extend the time to request a review of a Screening Decision where the Person or Owner demonstrates, on a balance of probabilities, the existence of extenuating circumstances that warrant the extension of time.

Where an extension of time is not granted by the Hearing Officer, the Screening Decision, Administrative Penalty, and any Administrative Fees, if applicable as modified in the Screening Decision, are deemed to be affirmed.

In the event that your Hearing Extension Request is approved, you will need to explain the reasons for disputing the Screening Decision. While you are not required to provide additional evidence, please note that the Hearing Officer can only make a decision based on the information presented to them. Submit copies of any documents that may assist as evidence.

This may include:

  • Photographs
  • Condominium letter/lease
  • Accessible parking permit
  • Medical note
  • Death certificate

If you are in financial hardship and require additional time to pay, please bring documents to support this claim. Documents may include:

  • Old Age Security
  • Canada Pension
  • Guaranteed Income Supplement
