Social Insurance Number and/or Birth Certificate applications
are available at the Customer Service Centre, Town Hall. The Town of Caledon regrets that we cannot process these applications, nor can we accept payment for them.

Birth Certificate Applications

Application forms are available on-line at Service Ontario for parents of children aged 8 and under, at Town Hall, from the Office of the Registrar General, and at Ontario Land Registry Offices. In general, please allow 14 weeks for processing.  

Birth Certificate Types

A birth certificate is a legal document needed to establish legal identity and will be used throughout your child’s life to obtain health services, a passport, old age pension and many other beneficial services. You can apply for different types of certificates:

Certificate – Includes basic information such as name, date and place of birth (2.5 x 3.75”).

Long Form (Certified)- Contains all registered information, including parents' information, and signatures and is a certified copy of the original record (8.5 x 14”).

Confirmation Letter- This letter states whether or not your records are on file. If you do not know the exact date of the event, choose a year and write it in the space provided for the date. A search will be conducted for an event that exactly matches the information provided, for that entire year, plus two years before and after, for a total of five years.

Entitlement to Birth Certificates

Anyone may apply for a Confirmation Letter. You may apply for a Birth Certificate or Long Form (Certified) if you are one of the following:
● The person named on the certificate and you are at least 13 years old
● A parent of the child named on the certificate and your name is on the live birth registration
● The closest next-of-kin, executor, or estate administrator, and the person named on the certificate of the deceased. You must provide proof of death (e.g. a death certificate or a funeral director's statement)

Birth Certificate Fees

Application forms and associated fees must be sent to the Office of the Registrar General.
First Birth Certificate - $25.00
Replacement Birth Certificate - $35.00
First Certified Copy of Birth - $35.00
Replacement Certified Copy of Birth - $45.00
Confirmation letter (5-year search) - $15.00 (a $15.00 charge applies to each additional 5-year block)
